It would have been nice if the tip would have included mention that the hook or some kind of washer needs to be placed on the screw prior to putting the toggle bolt into the wall so that the screw doesn’t simply fall through the hole in which is created by inserting the toggle bolt.

not proper wiring technique I’m afraid. you should wrao the end back into the d-hook before wh=rapping it back to the wire.
